To streamline the test request process, we have made our request forms available online.   To get a sample tested, just follow these simple steps.

1. Fill out the Request Form

2. Print the completed Request Form 

3. Send the sample and completed Request Form, via courier, to our Laboratory.

4. We will contact you upon receipt of the sample to complete the process.
